Gilled Midge Pupa
This pattern imitates the midge pupa just before emegence - you can see on the picture above of a real midge the white gills at the top of the head. A great technique to fish this pattern when the fish are rolling on emergers is behind a dry fly, applying floatant to the leader to the tippet until about 6 inches short of the midge, allowing it to drift  on or just below the surface film.
